目的比较地屈孕酮与黄体酮用于治疗先兆流产的临床效果。方法将研究期间的先兆流产患者119例随机分为试验组和对照组,试验组62例,给予口服地屈孕酮片治疗,对照组57例,给予黄体酮肌肉注射治疗,对两组患者治疗效果及围生儿结局进行综合比较。结果试验组保胎成功率为85.48%,高于对照组70.18%,差异有统计学意义(P<0.05);两组新生儿体重、治疗时间以及新生儿畸形比例比较差异无统计学意义(P>0.05);两组在治疗后6周、8周、10周的血清孕酮水平比较均差异无统计学意义(P>0.05);试验组平均治疗费多于对照组(P<0.05)、试验组不良反应发生率(4.84%)明显低于对照组(15.79%),差异均有统计学意义(P<0.05)。结论地屈孕酮治疗先兆流产较黄体酮有更高的保胎成功率,且安全性更高,缺点是价格稍偏贵,建议在临床逐步推广应用。

Objective To compare the clinical effects of dydrogesterone and progesterone in treatment of threatened abortion.Methods 119 cases of patients with threatened abortion were randomly divided into experimental group and control group during the study period. 62 cases in experimental group were treated with oral dydrogesterone tablets while 57 cases in the control group were given progestin muscle injection treatment. The treatment effect of the patients in two groups and the perinatal outcome were comprehensively compared. Results The success rate of miscarriage in the test group was 85. 48%,which was higher than that of the control group( 70. 18%,P < 0. 05); The neonatal weight,treatment time and the proportion of newborn malformation between two groups had no significant difference( P > 0. 05); The serum progesterone levels in the two groups after treatment of 6 weeks,8 weeks,10 weeks showed no obvious differences( P > 0. 05); The average cost of treatment in the test group was higher than that in the control group( P < 0. 05); The incidence rate of adverse reaction in test group was 4. 84%,which was significantly lower than that of the control group( 15. 79%)( P < 0. 05). Conclusions Dydrogesterone in treatment of threatened abortion has a higher success rate of miscarriage than progesterone and higher safety,but the disadvantage is a little expensive. It is suggested in the clinical popularizing application.
