目的 探讨基于互联网的正念减压疗法(mindfulness-based stress reduction,MBSR)对女性医护人员的潜在保护作用。方法 将2022年4月1日至2023年6月30日在浙江省某三级专科妇产科医院以自愿为原则纳入的89名女性医护人员作为研究对象,随机分为实验组和对照组。实验组予以8周的基于互联网的标准化的MBSR,对照组不进行干预。采用匹茨堡睡眠质量指数量表(Pittsburgh Sleep Quality Index,PSQI)、健康问卷抑郁量表(Patient Health Questionnaire-9,PHQ-9)、广泛性焦虑量表(Generalized Anxiety Disorder-7,GAD-7)、五因素正念度量表(the Five Facet Mindfulness Questionnaire,FFMQ)和积极消极情感量表(Positive and Negative Affect Schedule,PANAS)对两组进行评估,比较两组干预前后的差异。评估MBSR在正念状态、焦虑、抑郁、睡眠、积极消极情感方面的有效性,并探讨其在医院开展的可行性。结果 实验组和对照组干预前后变化值在PHQ-9得分(95%CI:-4.08~-0.29)、FFMQ总分(95%CI:4.95~14.96)及其观察维度得分(95%CI:3.30~8.95)和不行动维度得分(95%CI:0.93~5.26)以及消极情感得分(95%CI:-6.86~-0.07)等方面比较差异均有统计学意义(均P<0.05),而在PSQI总分(95%CI:-2.91~1.26)、GAD-7得分(95%CI:-2.13~0.61)及积极情感得分(95%CI:-0.66~5.52)等方面差异均无统计学意义(均P > 0.05)。结论 基于互联网的MBSR课程可以显著降低女性医护人员的抑郁症状,改善负性情绪倾向,提升正念水平。MBSR课程在医院开展具有很大的发展空间,值得进一步探索和实践。
Abstract:Objective To explore the potential protective effect of internet-based mindfulness-based stress reduction(MBSR) on the physical and mental health of female healthcare workers. Methods A total of 89 female healthcare workers who voluntarily enrolled in a tertiary specialized obstetrics and gynecology hospital in Zhejiang Province of China, from April 1, 2022, to June 30,2023,were randomly divided into an experimental group and a control group. The experimental group received standardized internet-based MBSR for 8 weeks, while the control group received no intervention. The Pittsburgh Sleep Quality Index(PSQI), Patient Health Questionnaire-9(PHQ-9), Generalized Anxiety Disorder-7(GAD-7), Five Facet Mindfulness Questionnaire(FFMQ) and Positive and Negative Affect Schedule(PANAS) were used for assessment. The differences in outcomes before and after intervention between the two groups were compared to evaluate the effectiveness in terms of mindfulness state, anxiety, depression, sleep quality, and positive and negative affects. Additionally, the feasibility of implementing the MBSR course in hospitals was explored. Results After the intervention, significant differences in the changes in scores were observed between the experimental group and the control group for the PHQ-9(95% CI:-4.08 to-0.29), total FFMQ score(95% CI:4.95 to 14.96), FFMQ observation subscale score(95% CI:3.30 to8.95) and FFMQ non-judging subscale score(95% CI:0.93 to 5.26), and and PANAS negative affect(NA) score(95% CI:-6.86 to-0.07)(all P < 0.05). In contrast, no statistically significant differences were found in the total PSQI score(95% CI:-2.91 to 1.26),GAD-7 score(95% CI:-2.13 to 0.61), and PANAS positive affect(PA) score(95% CI:-0.66 to 5.52) between the two groups(all P > 0.05). Conclusion Internet-based MBSR courses can significantly reduce depressive symptoms, improve negative emotional tendencies, and enhance the mindfulness level of female healthcare workers. MBSR courses have great development potential in hospitals and are worthy of further exploration and practice.
