目的了解武汉市某城区已婚妇女宫颈HPV感染状况和基因亚型分布情况,为该地区预防HPV感染和防治宫颈癌提供科学依据。方法采用原位杂交技术对已婚妇女的宫颈脱落细胞标本进行23种HPV基因分型检测。结果共对1983例已婚妇女的宫颈脱落细胞标本进行了检测,HPV总感染率为12.86%(阳性者255例)。HPV感染者集中在25~49岁年龄组(阳性者246例),占96.47%,各年龄组HPV感染率间差异有统计学意义(χ2=16.002,P<0.05);HPV感染者中82.35%为单一型,其中高危单一型感染占75.71%;混合型感染占17.65%,以双重感染为主(71.11%);基因亚型总检出率从高到低依次为16、58、18、11、56、33和52型,而混合型感染主要型别为18、58和16型。结论该地区HPV感染以16、18、58型为主,高发于25~49岁组妇女,针对25~49岁组妇女开展HPV感染检测和准确分型对宫颈病变的防治具有重要的意义。
Abstract:Objective To provide the science evidence for prevention and treatment of cervical cancer by investigating infection status and genotypes of human papillomavirus( HPV) among 1983 married women in one of districts in Wuhan,Hubei province.Methods In situ hybridization technology was utilized for the detection of 23 kinds of HPV genotypes in cervical exfoliated cell specimens from married women. Results In 1983 cases of subjects,the infection rate of HPV was 12. 86%( 255 HPV-positive women). The peak age of HPV infection ranged from 25 to 49 years old. Statistically significant difference was found in the HPV infection rate among all the age groups( χ2= 16. 002,P < 0. 05). Most of HPV-positive women( 82. 35%) were infected single type,including 75. 71% for high-risk,while double infections took priority of the multiple HPV-positive women( 17. 65%). HPV 16 was the most common type,and the other common types,in descending order of frequency,were type 58,18,11,56,33 and 52. The primary types of the multiple HPV infections were type 18,58 and 16. Conclusion The common genotypes of HPV-positive women were type 16,18 and 58 in this district. HPV infection often occurs in the women of the age group of 25-49 years. Test and genotype analysis of HPV play an important role in prevention and treatment of cervical lesions.
