甘肃省兰州市妊娠期妇女不同孕期甲状腺功能正常参考值范围的研究

摘要:

目的 观察正常妊娠妇女不同孕期甲状腺功能正常参考值范围及其动态变化。方法 本研究采用横断面研究方法,选取2017年12月1日至2018年11月30日在甘肃省兰州市妇幼保健院首次进行常规检查的妊娠期妇女进行筛查,经过招募后最终入选649例。通过发放调查问卷的方式,收集妊娠期妇女的基本资料。所有入选对象均抽取空腹静脉血,测量促甲状腺激素(thyroid stimulating hormone, TSH)、游离四碘甲状腺原氨酸(free tetraiodothyronine, FT4)、游离三碘甲状腺原氨酸(free triiodothyronine, FT3)、甲状腺过氧化物酶抗体(thyroid peroxidase antibody, TPO)和甲状腺球蛋白(thyroglobulin, TG)抗体值。结果 649例妊娠期妇女孕早、中、晚期TSH正常值范围分别为(0.409~4.284)uIU/mL、(0.898~4.514)uIU/mL和(1.252~4.526)uIU/mL;FT4的正常值范围分别为(8.851~18.060)pmol/L、(8.406~17.391)pmol/L和(8.402~17.922)pmol/L;FT3的正常值范围分别为(1.587~4.418)pg/mL、(1.895~4.612)pg/mL和(2.205~4.812)pg/mL。结论 本研究对建立兰州市妊娠期妇女特定时期血清TSH、FT4和FT3正常参考范围提供了依据,对保障妊娠期妇女风险管理、减轻疾病负担及提早预防胎儿的不良结局有重要意义。

Abstract:

Objective To observe the range of normal reference values of thyroid function and its dynamics among normal pregnant women in different periods. Methods A cross-sectional study was conducted from December 1 st, 2017 to November 30 th, 2018. Pregnant women who underwent first routine care were screened and 649 cases were recruited. Questionnaire survey was used to collect basic information from pregnant women. All the subjects tested thyroid stimulating hormone(TSH), free tetraiodothyronine(FT4), free triiodothyronine(FT3), thyroid peroxidase(TPO) antibody and thyroglobulin(TG) antibody values.Results The normal values of TSH was(0.409-4.284) uIU/mL,(0.898-4.514)uIU/mL, and(1.252-4.526)uIU/mL, and that of FT4 was(8.851-18.060)pmol/mL,(8.406-17.391) pmol/mL, and(8.402-17.922)pmol/mL, and that of FT3 was(1.587-4.418)pg/mL,(1.895-4.612)pg/mL and(2.205-4.812)pg/mL, respectively, among 649 pregnant women during early, middle, and late pregnancy. Conclusion This study provides evidence for establishing the normal reference ranges of serum TSH, FT4 and FT3 among women during specific periods of pregnancy at Lanzhou. It is meaningful to ensure risk management of pregnant women, reduction of disease burden and early prevention of adverse fetal outcomes.
