目的探讨血清VEGF、TNF-α、IL-1β、CA125联合检测在子宫内膜异位症诊断中的应用价值。方法选取2015年5月-2017年3月在湖南省妇幼保健院接受治疗的子宫内膜异位症患者为研究对象,并作为观察组。同时选取80例健康者作为对照组,比较两组患者之间及观察组不同疾病分期患者的血清中VEGF、TNF-α、IL-1β、CA125水平的差异,分析子宫内膜异位症患者CA125水平与VEGF、TNF-α、IL-1β水平的相关性。结果观察组患者血清VEGF、TNF-α、IL-1β和CA125水平明显高于对照组(t=-36.588、-45.027、-26.487、-111.105,P<0.001);Ⅲ~Ⅳ期子宫内膜异位症组患者的血清VEGF、TNF-α、IL-1β和CA125水平明显高于Ⅰ-Ⅱ期子宫内膜异位症组(t=-38.815、-7.697、-9.905、-21.326,P<0.001);子宫内膜异位症患者血清CA125水平与VEGF、TNF-α、IL-1β水平明显正相关(r=0.489、0.512、0.538,P=0.023、0.015、0.002);CA125对子宫内膜异位症诊断特异度和灵敏度分别为45.63、35.18,CA125联合VEGF、TNF-α、IL-1β对子宫内膜异位症诊断特异度和灵敏度较高,分别为65.32、55.86。结论子宫内膜异位症患者的血清VEGF、TNF-α、IL-1β、CA125水平较高,且与患者的临床分期密切相关,可作为临床诊断的重要指标。
Abstract:Objective Objective To explore the application value of serum VEGF,TNF alpha,IL-1 beta,CA125 joint detection in the diagnosis of endometriosis.Methods The patients with endometriosis treated in our hospital from May 2015 to March 2017 were selected as the research objects,and as the observation group,80 healthy persons were selected as control group.The difference in the levels of serum VEGF,TNF alpha,IL-1 beta and CA125 among patients with different diseases stage between the two groups was compared and observed.The correlation between the level of CA125 and the levels VEGF,TNF alpha and IL-1 beta among patients with endometriosis was analyzed.Results The levels of serum VEGF,TNF-α,IL-1 and CA125 in the observation group were significantly higher than those in the control group(t=-36.588,-45.027,-26.487,-111.105,respectively,P<0.001);The levels of serum VEGF,TNF-,IL-1 and CA125 were significantly higher in patients with stage IV endometriosis than those in group I and II(t=-38.815,-7.697,-9.905,-21.326,respectively,P<0.001);The level of serum CA125 was significantly positively correlated with the levels of VEGF,TNF-and IL-1 among patients with endometriosis(r=0.489,0.512,0.538,P=0.023,0.015,0.002,respectively).The specificity and sensitivity of CA125 in the diagnosis of endometriosis were 45.63 and 35.18,respectively.The sensitivity and sensitivity of CA125 combined with VEGF,TNF-alpha and IL-1 beta in endometriosis were 65.32 and 55.86,respectively.Conclusion The levels of serum VEGF,TNF alpha,IL-1 beta,and CA125 among patients with endometriosis are higher and they are closely related to the clinical stages of patients.Therefore,the levels of serum VEGF,TNF alpha,IL-1 beta,and CA125 can be used as important indicators for clinical diagnosis.
